In accordance with Texas House Bill 4224 (89th Regular Session) and Section 181.105 of the Texas Health and Safety Code, behavioral health providers in Texas are required to make the following information publicly available.
How to Request Your Health Care Records
Clients have the right to request access to their health care records.
To request your records:
1. Submit a written request by email to angel@beginfromwithincounseling.com or through the secure client portal
2. Include your full name, date of birth, and the specific records you are requesting 3. Include a copy of a valid photo ID for identity verification
4. Requests may be subject to reasonable fees as permitted by law
5. Records will be provided within the timeframe required by Texas law (typically within 15 business days)
